EMS Lecture Series Registration Agreement

It is hoped that participants will find it easier and more convenient to renew their certification by participating in on-going continuing education activities. A New York State Certified Instructor Coordinator (CIC) must oversee all "Core Refresher Training" activity. The individual's Agency CIC bears sole responsibility for the content of these sessions. Continuing education credit can only be received for time the individual actually participates in the activity. The Albany Medical Center Hospital and/or a designee will conduct audits of randomly selected participants to review and verify their participation in both the video conference and post-testing continuing education activities. Participation, computer TCP/IP address records and other documentation will be reviewed for each item video/post-test listed in the participant's name. Some interviews may be scheduled with participants and/or others involved in the video * distance learning continuing education program.

I understand that if I have any questions about the content of these programs in general, I can contact the Albany Med Emergency Outreach Coordinator. I agree to abide by all regulations and policies set-forth by Albany Medical Center Hospital, REMO and NYS Bureau of EMS as they relate to this program.

The Albany Med Program (the "EMS Lecture Series") is designed to recognize personal knowledge with respect to EMS Education/curriculum. Albany Med has created a series of post-tests designed to objectively measure individual knowledge with respect to an EMS subject. The CEU conferred by the CME Program delivers its greatest value to you only as long as the Test is secure and continues to operate as a measure of personal knowledge. All candidates for AMC EMS Lecture CME Certification are required to read and click through the C Agreement. Albany Med requires all candidates to comply with all testing regulations imposed by AMC or it's designee. AMC has a two-week minimum waiting period before a candidate may attempt to re-take any test. In addition, a candidate may not attempt to take a test more than three (3) times within any calendar year without written permission from the AMC Emergency Medicine Outreach Coordinator.

The content of all tests and test materials are proprietary and confidential information of AMC. Candidates may not disclose or share any of the content of these materials; nor can candidates ask for, write (in any media), publish, or otherwise disclose any test questions or answers. Candidates may not forge certificates, misuse their user ID or password in the AMC EMS Lecture CME Certification Candidate Database. AMC might revoke any AMC Testing status or other rights previously conferred on any candidate, and may permanently bar candidates from participating in the AMC EMS Lecture CME Certification Program, if AMC (Any regional program agency, NYSDOH BEMS) determines that a candidate has engaged in any misconduct with respect to the AMC EMS Lecture Certification Program, including, but not limited to the prohibited conduct outlined in this policy their individual's certifications may be in jeopardy.
